Consistent with the Americans with Disabilities Act (ADA), the University of Utah provides reasonable accommodations to qualified individuals with disabilities. The University's Human Resources Management Department (UHRM) is charged with determining reasonable accommodations under the ADA. Requests for accommodation are reviewed on a case-by-case basis with the help of the individual's home department leader(s) with a need to know, and as needed to process the request.
Under Title I of the ADA, if an employee/faculty is experiencing barriers at work, due to a disability that impacts their ability to perform job functions or access benefits of employment, they can request an accommodation to help remove such barrier(s).

If you wish to submit supporting medical documentation along with your Accommodation Request Form, you may use this medical inquiry form. This medical inquiry form is an example of the type of information that may be needed to process your accommodation request. Your medical provider can complete the attached form, or provide any documentation that describes the nature, severity, and duration of your impairment, the activity or activities that the impairment limits, the extent to which the impairment limits your ability to perform the activity or activities, and should also substantiate why the requested reasonable accommodation is needed.
After you complete your submission, UHRM will notify you if additional information is needed to evaluate your request, will discuss your request with your home department leader(s) (with a need-to-know basis), and will inform you of any accommodation(s) that can be provided.
