Veterans & Service Members who are applicants:
Any veteran applicant who is interested in claiming Veterans' Preference in our application process should complete
and submit the
Veterans'
Preference Request Form and attach one of the forms listed below:
- NGB22, DD-214 (Copy 2, 4 or 6), or other applicable document- must include length of service, character of service and type of separation, OR
- Certification of Active Service In Lieu of a DD-214.
The requirements to obtain veterans’ preference are listed within the
Veterans' Preference Addendum.
Also listed in this document is the process used by the University regarding Veteran applicants.
The preferred way to deliver documents is to submit the online
Veterans' Preference Request Form. However, you may
also email the completed Addendum and applicable forms to
Employment@Utah.Edu,
or can be delivered to Human Resources at 250 East 200 South, Suite 125 Salt Lake City, Utah 84111.

Contact Utah Department of Veterans Affairs at 801-326-2372 to obtain further instructions for obtaining a copy of a DD-214 showing the status of your discharge.

To identify your service as a Veteran, please login to CIS, go to the "Employee" tab, under the "Employee Self Service Apps" section select "Personal Bio/Demo Information" and update your information under "Veteran Status."
